Who Can Make An Appointment?
-
First-time adult applicants age 18 and older
-
Adults who do not qualify for renewal because their passport was:
-
lost, stolen, or damaged
-
issued over 10 years ago, or
-
issued when they were under age 16
-
-
Children under the age of 15, with approval of all parents/legal guardians
-
Teens aged 16-17, with the approval of at least one parent/legal guardian

What Do I Need to Bring To My Appointment?
-
DS-11 form
-
Passport photo
-
Proof of U.S. citizenship, and black and white photocopy
-
Official state or federal ID with photo, and black and white photocopy (front and back)
-
Check or money order (for application fee to “U.S. Department of State”) PER APPLICATION
-
Check or money order (for execution fee to Marin County Law Library)
-
Any additional documents (legal name changes, requests for exceptions to photo requirements, lost/stolen passport forms, etc)
