Who Can Make An Appointment?

  • First-time adult applicants age 18 and older

  • Adults who do not qualify for renewal because their passport was:

    • lost, stolen, or damaged

    • issued over 10 years ago, or

    • issued when they were under age 16

  • Children under the age of 15, with approval of all parents/legal guardians ​

  • Teens aged 16-17, with the approval of at least one parent/legal guardian

What Do I Need to Bring To My Appointment?

  1. DS-11 form

  2. Passport photo

  3. Proof of U.S. citizenship, and black and white photocopy

  4. Official state or federal ID with photo, and black and white photocopy (front and back)

  5. Check or money order (for application fee to “U.S. Department of State”) PER APPLICATION

  6. Check or money order (for execution fee to Marin County Law Library)

  7. Any additional documents (legal name changes, requests for exceptions to photo requirements, lost/stolen passport forms, etc)
